Client work we perform under contract, including any work involving patient information, is governed by the client agreement and, where applicable, a Business Associate Agreement.

• Booking information: when you book a call, Calendly collects your name, email, chosen time and any answers you provide, and shares them with us.

Please do not send patient or health information through this website. Our forms are not designed to receive protected health information. If you need to share it, we will arrange a secure channel first.
